Call or Text us at (815) 609-9480 to schedule your appointment.
STEP 2
Collect vaccine records
Gather all your vaccine records. Even if another language, you can bring a translation.

No vaccines, no problem! We can provide any missing vaccines at your appointment.

What happens at the visit
Dr. Iskander does the I-693 exam in the office. If you need labs or missing vaccines, we do those here at the same visit.
After your physical exam and all labs and vaccines have been administered our turnaround is quick. We will give you a call within 7 days (includes weekends) to come review and pick-up your completed documents.
We will provide you a sealed envelope, which you should not open. And we will also provide a copy for your own records.
